The Home Health Admission RN is responsible for initiating patient care through Start of Care (SOC) admissions, conducting supervisory visits for LPNs in their assigned region every 30 days, and completing recertifications following 30 days of care or post-hospitalization. This role also includes providing skilled nursing visits.
